Medieval torture

The one I find particularly horrific isn't on your list but George Orwell and Bret Easton Ellis have both used it.

And here I was taken aback by the idea of doing this sort of research. I've been shirking my dooty. :(
 
I like the Heretic's Fork because it can be used as punishment but it visually displays how motivated a person will be to provide you the information you want with or without causing damage. You can even modify the device so that the two sets of forks can slowly be wound away from each other.
