The Forgotten Realms, i know its Tolkienesce but i play the Baldur's Gate games and Neverwinter Nights, and love Salvatore so definetly the Forgotten Realms.

I love Forgotten Realms and other Dungeons and Dragons settings,but I also agree with Katzen that Osten Ard is great too. I am on Stone of Farewell by Tad Williams right now and I wish I have had more time recently to finish it.

If you are going to list Midkemia you have to list Kelewan - Daughter of the Empire, Servant of the Empire and Mistress of the Empire
