Help me!! meaning of a passage

Please, please help me with the meaning of a passage. i have no effin idea about what was being talked about:

Book: Sula, by Toni Morrison
Page 97:

"You puttin' it on, Nel. Jude must be wore out."
"Jude must be wore out? You don't care nothin' 'bout my back, do you?"
"Is that where it's at, in your back?"
"Hah! Jude thinks it's everywhere."
"He's right, it is everywhere. Just be glad he found it ..."

What the fu-- does "wore out" mean in this context? What's happening to Nel's back? If you have read Toni Morrison (or thinks this passage is obvious) Please tell me what its meaning is. Thank you so much!
